The Football Association is reportedly investigating the tragic death of Maddy Cusack after new information was provided by her family.
The Sheffield United star died at his home in Derbyshire on 20 September, with police saying there were no suspicious circumstances surrounding his death.
In October, Blades appointed an independent third party to conduct a formal investigation.
According to the Daily Mail, Cusack’s family gave Sheffield United a list of names of people they felt needed to speak to in the independent investigation.

However, they were reportedly ‘disappointed’ that some names were not contacted until the investigation was concluded.
The list has now been sent to the FA, who are ready to review the new information.
A third-party investigation found no evidence of wrongdoing but, earlier this month, the FA confirmed they were reviewing that process.
It has been claimed that FA officials met with Cusack’s family this week, having had ‘positive’ talks.
Speaking recently, Cusack’s father claimed that ‘if the club had not appointed Jonathan Morgan as coach she would still be with us.’
It has been alleged that Morgan called the Sheffield United star a ‘psycho’ and made complaints about his weight before his death. He strongly denies any wrongdoing.
David Cusack said: “There were a number of factors in the end that troubled her, but they all stemmed from the relationship with J.M. [Jonathan Morgan],
“As he told us (his family), the origin of every issue was in the appointment of JM.
“We know she would still be with us if she had not been recruited. Her text messages and conversations support this.”
Cusack had previously worked with Morgan at Leicester.
Morgan will continue to serve as manager of the club’s women’s team.
In December, Sheffield United released a statement on the findings of the investigation.
“At the request of Maddie’s family and with their cooperation, we appointed an independent third party to formally investigate the concerns raised by the family about the processes and conduct at the club,” the statement said.
“Following a thorough review of the valuable input given by Maddie’s family and each key witness provided by the club, the investigation has found no evidence of wrongdoing.
“The Club is always looking for ways to develop and will consider the findings and recommendations coming from the investigation to consider how processes and policies can be improved.

“Following Maddie’s death, the club provided staff with the opportunity to engage with additional external support.

Cusack’s mother Deborah said at a memorial service in October that football had broken her daughter’s spirit.
